Panorama has seen documents, obtained by Jade through an FOI request, from the HSE's investigation into the Airmaxx 360 accident in Hull.

In its FoI response, Highland Council admitted that not all its schools were recording racist incidents on the school recording system, SEEMIS.

The FOI also asked the length of service of the longest serving agency worker employed by the council and was told that they had been doing the job for 13 years.

FOI laws came into force in the UK in January 2005 and allow anyone to apply to government or public bodies to see information, such as crime statistics or details on expenditure.

Our investigation revealed the CRU did not follow the so-called "applicant-blind principle" when responding to FOI requests.
